- Please fill in all boxes as completely as possible with appropriate information
- For shipments originating outside of the U.S., please reflect the following information on all documents: “HTS code 9801.00.1012” -AND- “MADE IN USA” -AND- “Goods of U.S. origin being returned for repair.”
- If an estimate is required, an AMI representative will contact you with evaluation results and estimated cost for non-warranty repairs. Repairs will begin upon receipt of a Purchase Order for these charges. Estimates will generally increase repair time. An evaluation fee will be charged if the customer decides not to have the unit repaired.
- Please allow 2-3 weeks for evaluation of repair. In many cases, special set-ups are required.
- Warranty period is one year from the date of the original shipment.
- If repair is non-warranty, return freight charges will be prepaid by AMI and added to invoice.
