    1. Please fill in all boxes as completely as possible with appropriate information
    2. For shipments originating outside of the U.S., please reflect the following information on all documents: “HTS code 9801.00.1012” -AND- “MADE IN USA” -AND- “Goods of U.S. origin being returned for repair.”
    3. If an estimate is required, an AMI representative will contact you with evaluation results and estimated cost for non-warranty repairs. Repairs will begin upon receipt of a Purchase Order for these charges. Estimates will generally increase repair time. An evaluation fee will be charged if the customer decides not to have the unit repaired.
    4. Please allow 2-3 weeks for evaluation of repair. In many cases, special set-ups are required.
    5. Warranty period is one year from the date of the original shipment.
    6. If repair is non-warranty, return freight charges will be prepaid by AMI and added to invoice.
      If repair is covered under warranty, return freight charges via UPS ground (or equivalent) will be paid by AMI.
